Step 1: Submit appointment form

Enter applicant, passport issue/expiry, visa, and city details in one complete form.

Step 2

After form submission, upload both slips: PKR 4,000 appointment fee and PKR 23,000 medical fee.

Step 3

Medical center assignment is handled through Gulf Health Council workflow after verification.

Step 4

Use booking reference or passport number to track pending, under-review, confirmed, or rejected status.
